Leading discharge planning
Leading and coordinating overnight and early discharge planning to optimise patient flow🤝
Collaborating with teams
Collaborating with treating teams and multidisciplinary services to identify suitable patients for transfer to Transit Care Hub🔄
Supporting quality improvement
Supporting quality improvement, change management and service development initiatives within the Patient Flow ProgramWhat we're looking for

Valid registration as a Registered Nurse in Australia.View
