You'll be responsible for

Providing relief case management across multiple client portfolios

Stepping in to manage various client needs and ensure continuity of care.

Conducting client check-ins, reviews and assessments

Regularly assessing client wellbeing and safety to ensure high-quality care.

Supporting the delivery and coordination of services

Ensuring that care plans are followed and services are delivered as per client needs.

Key criteria

👥

Previous experience in care roles

Demonstrated experience as a Care Partner or Case Manager.

Certificate IV in Aged Care

Possession of Certificate IV in Aged Care or equivalent.

Current driver's licence

Valid driver's licence and access to a reliable vehicle.
