You'll be responsible for

Providing intensive case management
Provide intensive case management, assessment, planning, monitoring and crisis intervention to clients transitioning from prison to the community.📝
Conducting assessments
Conduct assessments and develop individual service plans to support clients' needs and goals.🤝
Collaborating with partners
Work collaboratively with multiple partners, including prison staff and community organizations, to ensure comprehensive support for clients.Key criteria
🎓
Tertiary qualifications in relevant fields
Possession of qualifications in Social Work, Psychology, or related disciplines.View
🕒
3+ years in community services
Experience in community services or support and housing roles.View

Strong case management skills
Demonstrated ability to support marginalised clients with complex needs.View
