You'll be responsible for

Providing intensive case management

Provide intensive case management, assessment, planning, monitoring and crisis intervention to clients transitioning from prison to the community.

Conducting assessments

Conduct assessments and develop individual service plans to support clients' needs and goals.

Collaborating with partners

Work collaboratively with multiple partners, including prison staff and community organizations, to ensure comprehensive support for clients.

Key criteria

🎓

Tertiary qualifications in relevant fields

Possession of qualifications in Social Work, Psychology, or related disciplines.

Experience in community services or support and housing roles.
