You'll be responsible for

Providing person-centred case management
Support young people experiencing or at risk of homelessness through intake, assessment, case planning, and ongoing support.🏠
Advocating for and coordinating access to housing and services

Maintaining accurate records and contributing to team initiatives

Diploma in Community Services or equivalent
