You'll be responsible for

Acting as the first point of contact

Serve as the initial point of contact for elderly and vulnerable clients, providing support and information.

Liaising with stakeholders

Communicate with medical centres, hospitals, and other stakeholders to coordinate in-home support services.

Connecting clients with services

Assist clients in accessing services such as personal care and domestic support, ensuring their needs are met.
