Coordinate and manage Rehab at Home programs for clients discharging from hospital to home-based care following orthopaedic surgery or deconditioning post hospital admission.

Supporting and communicating with clients remotely

Support and communicate with clients remotely to achieve their personal and clinical rehabilitation goals within their home environment.

Managing clinical case escalations

Manage clinical case escalations and variations to expected clinical progress while maintaining relationships with multiple stakeholders.

Key criteria

🎓

Physiotherapy or nursing degree

Possession of a relevant degree and AHPRA registration.

Demonstrated experience collaborating within diverse healthcare teams.
