Delivering expert clinical assessment and consultancy for patients with complex chronic heart failure and chronic disease management.📝
Developing individualised care management plans
Creating, implementing, and evaluating care management plans in partnership with patients, carers, and multidisciplinary teams.🏥
Leading evidence-based cardiac care practices
Leading the development and implementation of evidence-based practices across services to improve patient outcomes.Key criteria

Registered Nurse with AHPRA registration
Possession of current AHPRA registration as a Registered Nurse.View
