You'll be responsible for

Delivering vital clinical support to nursing and home care teams across a 7-day per week service.🏥
Managing client care
Supporting hospital avoidance initiatives and managing clients with injuries, illnesses, or exacerbations of chronic and complex health conditions in the community.📚
