An exciting opportunity awaits a highly motivated and enthusiastic Registered Nurse to join the Aged Care Services in Emergency Team (ASET) at Liverpool Hospital. This job is all about providing specialised care and comprehensive assessments to older individuals in the Emergency Department, aiming to improve health outcomes and reduce hospital stays. The team thrives on collaboration, innovation, and a commitment to person-centred care, making a real difference in the lives of older patients.

You'll be responsible for

📋

Leading comprehensive assessments

Lead comprehensive geriatric assessments in the ED to determine safe and timely pathways for older patients, ensuring decisions support discharge, short-stay admission, or escalation to acute care.
🚦

Driving effective care planning

Drive effective care planning and patient flow by creating tailored, person-centred plans that enable safe discharge or appropriate admission, reducing congestion and improving ED efficiency.
⚠️

Championing early identification

Champion early identification and escalation of geriatric syndromes such as delirium, frailty, falls risk, and functional decline working closely with ARIA and Advanced Aged Care teams to expedite assessment and reduce ED delays.
