You'll be responsible for

Providing national Duty Care Management support
Support the Support at Home program by managing client escalations and ensuring continuity of service.🔗
Liaising with Care Managers and teams
Facilitate effective client handover and hand-backs to ensure seamless service delivery.📄
Maintaining high standards of documentation
Ensure compliance with required processes and documentation standards to uphold quality care.What we're looking for
