You'll be responsible for

Managing referrals and assessing eligibility

Overseeing the referral process to ensure timely and accurate intake of clients into community health services.

Collecting and entering demographic and clinical information

Accurately inputting data into health information systems to support clinical triage.

Collaborating with the team to improve service efficiency

Working together with colleagues to manage workflow and enhance the intake process.

Skills you'll need

💻

High level computer knowledge in databases or health information systems

Ability to navigate and utilize systems such as iPM, Cerner, and Microsoft Word & Excel applications effectively.

Effective customer service and communication skills

Strong verbal, written, and interpersonal skills to develop and maintain relationships with clients, team members, and clinicians.

Excellent time management and organisation skills

Ability to manage time effectively, pay attention to detail, and work with minimal supervision while handling diverse tasks.
