You'll be responsible for

Conducting assessments
Conduct comprehensive ADL assessments to evaluate an individual's functional abilities and support needs within the home and/or community to promote their independence.📞
Liaising with stakeholders
Liaise with employers, case managers, medical professionals, and insurers to provide clinical updates and ensure alignment of rehabilitation goals.📄
Preparing reports
