Coordinating care using WMG-E platform
Coordinate care using the Watch Me Grow–Electronic (WMG-E) platform, ensuring timely, integrated support through intake, assessment, referrals, and follow-up.📝
Developing individualised care plans
Develop and monitor individualised care plans in collaboration with service providers and partners.Key criteria
