Responding to the needs of the identified young people and families to assist with engagement and personalised learning plans in partnership with school communities and external stakeholders.💪
Building resilience
Building resilience through facilitation of referrals to other allied health professionals, and service and care coordination for the purpose of assisting young persons and their family's wellbeing and mental health.📝
Providing professional assessments
Providing professional mental health and social welfare assessment and interventions and follow-up support of the identified young people and families as required.What we're looking for
📚
Tertiary qualifications in Social Work
Possession of relevant qualifications and AASW membership is essential.View fit

Experience in complex case management
Demonstrated experience managing complex cases in educational settings.View fit
