Conducting comprehensive psychosocial and functional assessments, determining eligibility for home care packages or residential care, and completing support care plans.❤️

Delivering patient-centred care, building health literacy and promoting self-management.🤝
Participating in multi-disciplinary service approach
Actively participating and where appropriate, leading family meetings and contributing to the evaluation of the Social Work service and Clinical Specialty.Key criteria

Social work degree
Possession of a degree in social work from an accredited institution.View
