Supporting chronic disease management
Focusing on prevention and management of chronic diseases for Aboriginal clients during their journey through custody and back into the community.Key criteria

Certificate IV in Aboriginal Primary Health Care
Holds a Certificate IV in Aboriginal Primary Health Care (Practice) with AHPRA registration.View

Certificate III in Aboriginal Primary Health Care
Holds or aspires to hold a minimum Certificate III qualification in Aboriginal Primary Health Care.View
🚗
NSW driver's licence
Eligible to drive in NSW with a clean driving record.View
