Providing case management support
Delivering high-quality case management support including assessment, case planning, reviews, and case closure.â¤ď¸
Delivering sensitive support
Providing holistic, sensitive support that responds to individual needs and enables clients to achieve case plan goals.đ
Maintaining client records
Keeping accurate client records, reporting, and data collection in line with funding and accreditation requirements.Key criteria

Tertiary qualification
Relevant qualification in Social Work or community services.View
