About the job

As a Case Manager in the Housing Pathways Program, you will have the opportunity to support court-referred participants in addressing issues that contribute to their risk of homelessness. Your work will play a vital role in reducing ongoing homelessness and re-offending by providing targeted, trauma-informed support. The team values collaboration and strives to create a supportive environment where everyone can thrive.

You'll be responsible for

Assessing housing needs

Assess immediate and long-term housing needs for clients exiting court or bail environments.

Connecting clients

Connect clients with appropriate housing options including crisis accommodation, transitional housing, rooming houses, and social housing.
📝

Developing case plans

Develop and monitor individual case plans using a trauma-informed, client-led approach.

Key criteria

🎓

Tertiary qualification in social work

Degree or significant experience in community services field.
