About the job

As a Case Worker for the Home Access for people with Disability program, you will play a vital role in helping individuals with disabilities secure stable and suitable housing. This position is all about making a meaningful difference in people's lives through collaboration and support. The team thrives on empowerment, growth, and a shared commitment to enhancing the quality of life for those they serve.

You'll be responsible for

🏠

Assessing needs and developing housing plans

Working closely with clients to understand their unique situations and create tailored housing solutions.
🤝

Coordinating with support services and landlords

Building partnerships to facilitate smooth transitions into suitable housing for clients.
🛠️

Providing practical assistance with tenancy and accessibility goals

Offering hands-on support to help clients achieve their housing objectives and improve their living conditions.

Key criteria

🏠

Minimum Cert IV or Diploma

Possession of a minimum Certificate IV in Human Services or related field.

2+ years experience

Demonstrated experience working in disability or human services for at least 2 years.

Valid Blue Card required for working with children.
