Provide assessment, counselling, and case management to clients experiencing acute and chronic conditions.🤝
Collaborating across programs
Work collaboratively across specialist programs including Aged Care, Palliative Care, Chronic Disease Programs, and Integrated Care Coordination.👥
Supervising and mentoring
Supervise, mentor, and provide professional development for Level 1/2 social workers and students.What we're looking for

Degree in Social Work
Possession of an accredited Social Work degree and AASW membership eligibility.View fit

Extensive clinical experience
Demonstrated experience in community settings across various conditions.View fit
