You'll be responsible for

Acting as the first point of contact
Serve as the first point of contact for elderly and vulnerable clients, ensuring they receive the support they need.🏥
Liaising with stakeholders
Communicate with medical centres, hospitals, and other stakeholders to set up in-home support services.🔗
Connecting clients with services
Assist clients in accessing various services such as personal care and domestic support.What we're looking for
