You'll be responsible for

Providing person led support

Delivering person led support to individuals with disability, enabling them to live a meaningful, self-directed life in accordance with their goals and preferences.
🌍

Supporting community participation

Facilitating access to social, recreational, cultural, work and educational opportunities, fostering relationships and valued roles in the community.

Maintaining documentation

Keeping accurate, timely and confidential documentation, including progress notes and incident reports, in accordance with organisational and regulatory standards.
