You'll be responsible for

Providing person led support

Delivering person led support to people with disability, enabling them to live a meaningful, self-directed life consistent with their goals and preferences.

Supporting community participation

Supporting people to exercise choice and control in all aspects of their daily lives, encouraging decision making, self-expression and participation in activities that are meaningful to them.

Maintaining documentation

Maintaining accurate, timely and confidential documentation, including progress notes, incident reports and other required records in accordance with organisational and regulatory standards.
