About the job

As an Initial Assessment & Planning Worker, you will be the first point of contact for individuals and families facing homelessness or at risk of it. Your compassionate approach will help guide them through their options and access vital support services. The team values collaboration and a person-centred experience, ensuring that every interaction is meaningful and impactful.

You'll be responsible for

🤝

Providing compassionate intake and assessment

Delivering compassionate intake, assessment, information, and referral services to individuals and families experiencing homelessness or housing crisis.
🔍

Conducting comprehensive assessments

Carrying out comprehensive assessments, including risk assessments, to identify client needs and develop appropriate support pathways.

Building relationships with community services

Establishing and maintaining strong relationships with community services and support networks to connect clients with the right resources.

Diploma of Community Services

Possession of a minimum Diploma of Community Services qualification.
