Delivering intensive case management

Providing intensive case management and transition support for students at risk of disengagement.

Establishing partnerships

Building effective partnerships with stakeholders, including families and community organizations.

Coordinating individual plans

Facilitating and leading the coordination of individual student plans and transition plans.

Experience in case management

Demonstrated experience delivering intensive case management for at-risk youth.
