You'll be responsible for

Playing a pivotal role in delivering holistic, person-centred care as part of a multidisciplinary team in the Obstetrics, Neonatology and Paediatric Service.

Supporting complex discharge planning

Supporting complex discharge planning and case management for safety and risk assessments and grief and bereavement care during pregnancy.

Collaborating with stakeholders

Collaborating closely with medical, nursing, allied health, and external stakeholder teams to ensure integrated care delivery and safety planning.

Key criteria

5+ years clinical social work experience

Demonstrated experience in a relevant specialty area is essential.
