Providing person-centered care
The Nurse Practitioner provides person-centered care at an advanced nursing practice level incorporating diagnostic and treatment interventions using a high level of knowledge, skills, and autonomy to achieve people-focused quality care.🤝
Collaborating with multidisciplinary teams
Working in partnership with medical officers and multidisciplinary teams to deliver holistic integrated care in line with NSW Health Hospital in the Home PD2025_004.📈
Participating in professional development
Evidence of participation in own professional development, a commitment to maintaining competence and enhancement of skills, and willingness to apply and share professional expertise as a resource person and mentor others within the field of practice.Key criteria

Registered Nurse with AHPRA endorsement
Possession of a valid AHPRA endorsement as a Nurse Practitioner.View

5+ years nursing experience
Demonstrated experience with at least 5 years post-graduate nursing.View
