You'll be responsible for

Delivering case management

Deliver trauma-informed, strengths-based case management and individual support to service users to address identified needs and achieve recovery goals.

Developing case plans

Develop, implement, and review individualised case plans, including goal setting and tailored support strategies that promote recovery and independence.
🤝

Collaborating with stakeholders

Work collaboratively with intake teams and stakeholders to complete required documentation, including assessments and self-needs reports.

Key criteria

📜

Degree in relevant field

Degree in Social Work, Human Services, or equivalent experience.
