You'll be responsible for

Providing patient-centered care
Working as a member of the Specialist Community Palliative Care Team to provide care for people with advanced life-limiting illnesses, their families, and carers.🔗
Collaborating with health professionals
Liaising with other palliative care health professionals to ensure comprehensive care across various settings.👥
Participating in a multidisciplinary team
Functioning as part of a multidisciplinary team, including primary care, specialist services, and community-based services.What we're looking for

Current AHPRA registration
Valid registration as a Registered Nurse with AHPRA is essential.View fit
