You'll be responsible for

Conducting in-home assessments
Performing assessments, reviews, and consultations for Home Care Package clients to ensure their needs are met.📋
Developing care plans
Creating, implementing, and regularly reviewing individualized, goal-based care plans in line with HCP guidelines.📊
Overseeing client expenditure
Managing client budgets and ensuring compliance with funding rules while aligning with client goals.Key criteria

Registered Nurse or Enrolled Nurse qualifications
Possession of RN or EN/EEN qualifications is preferred.View

Home Care Package management experience
Demonstrated experience managing Home Care Packages effectively.View
