Ensure all client details and case notes are accurately maintained and updated.

Engaging in ongoing care discussions and evaluations

Collaborate with the care team to assess and review client goals and outcomes.

Key criteria

Certificate III in Aged Care or equivalent

Essential qualification for providing personal care services.
