You'll be responsible for

Supporting transitions
Be accountable for assigned cases to progress transition plans, including undertaking appropriate case management, advocacy and support.📢
Communicating expectations
Communicate expectations of the transition program with students and stakeholders to develop their understanding of career and learning pathways.🤝
Engaging with students
Engage with students in good faith to identify their interests and recommend the best fit transition plan.Key criteria
