The Transition Pathways Officer is an exciting opportunity to work across a cluster of State High Schools in the Sunshine Coast area, focusing on maximizing the transition outcomes for students at risk of not completing Year 12. This role plays a crucial part in supporting schools and students by developing effective strategies and programs that ensure young people remain engaged in education or training. You'll be responsible for
📚
Supporting schools in policy development
Supporting schools to interpret, develop, refine and enact departmental policies, practices and procedures relating to students exiting school prior to the completion of Year 12.🤝
Providing case management and advocacy
Providing appropriate case management, advocacy and support to assist young people and their families in their transition into viable education, training, or employment.🌐
Building partnerships with stakeholders
Forging links with providers of employment services, community agencies, training organisations and employers to create opportunities for young people.What we're looking for
