Transition Pathways Officers play a vital role in supporting young people as they navigate their journey back to school or towards employment and training. By working closely with schools and stakeholders, these officers help ensure that at-risk students have the resources and guidance they need to succeed. The team values collaboration, effective communication, and a commitment to making a positive impact in the lives of young people.

You'll be responsible for

Supporting schools

Support schools to develop and refine departmental policies and practices for students exiting school before Year 12.

Providing support

Provide appropriate support and intervention to young people and their families for successful transitions.

Forging links

Forge links with employment services and community agencies to create opportunities for young people.

Experience in case management

Demonstrated experience managing cases for at-risk students.
