About the job

As an AOD Residential Case Manager, you will have the opportunity to deliver flexible, participant-centred, recovery-focused case management within a residential AOD treatment program. Your work will be essential in supporting participants to build autonomy, strengthen coping skills, and achieve meaningful recovery outcomes. You'll be responsible for

Building therapeutic relationships

Building therapeutic relationships that promote autonomy, hope and self-efficacy using strengths-based, recovery-oriented practice
📝

Completing holistic assessments

Completing holistic assessments to identify participant needs, strengths, risks and treatment goals
📋

Developing care plans

Developing and delivering care plans including treatment matching, outcome monitoring, aftercare and exit planning

AOD Skill Set certification

Possession of AOD Skill Set (CHCSS00093) is essential.
