You'll be responsible for

Conducting in-home assessments

Performing assessments, reviews, and consultations for Home Care Package clients to determine their needs.

Developing and implementing care plans

Creating individualized, goal-based care plans in line with Home Care Package guidelines.

Monitoring client wellbeing

Regularly checking on clients' needs and managing clinical and non-clinical risks to ensure their safety and comfort.

Key criteria

Registered Nurse or Enrolled Nurse qualifications

Possession of RN or EN/EEN qualifications is preferred.

Home Care Package management experience

Demonstrated experience managing Home Care Packages effectively.

Knowledge of HCP guidelines and compliance

Strong understanding of HCP funding, budgets, and compliance.
