You'll be responsible for

Implementing case plans with active client participation

Working collaboratively with clients to create and execute personalized case plans that address their unique needs.

Establishing effective relationships with clients

Building trust and rapport with clients to encourage their active participation in the support process.

Providing case management support to clients

Offering ongoing support and guidance to clients as they navigate their challenges and work towards their goals.

Key criteria

🎓

Degree in social work

Bachelor's degree in Social Work or related field required.
