Using personal journey to build trusting relationships and provide one-to-one and group-based support to individuals at risk of suicide or experiencing emotional crisis.

Identifying consumers

Identifying consumers at risk of admission to hospital and providing Step-Up care communication with staff in outreach teams for crisis follow up.

Documenting contacts

Documenting consumer contact using a non-clinical, strengths-based format that is integrated into eMR.
