You'll be responsible for

Leading comprehensive assessments
Lead comprehensive geriatric assessments in the ED to determine safe and timely pathways for older patients, ensuring decisions support discharge, short-stay admission, or escalation to acute care.📝
Driving care planning
Drive effective care planning and patient flow by creating tailored, person-centred plans that enable safe discharge or appropriate admission, reducing congestion and improving ED efficiency.⚠️
Championing early identification
Champion early identification and escalation of geriatric syndromes such as delirium, frailty, falls risk, and functional decline, working closely with ARIA and Advanced Aged Care teams.Key criteria

Registered Nurse qualification
Possession of a valid Registered Nurse license for clinical practice.View
