You'll be responsible for

Completing Home Support Assessments
Conducting assessments using the Integrated Assessment Tool (IAT) in line with guidelines and quality requirements.🔗
Coordinating client referrals
Working as part of the Aged Care Assessment - Grampians Team to link clients with appropriate service providers.🤝
Supporting coordinated services delivery
Ensuring transparency and coordination through the centralised client record and My Aged Care Gateway.Key criteria

Certificate IV or Diploma
Possession of relevant qualification in aged care sector.View
