You'll be responsible for

Coordinating services

Using a person-centred approach to assist in identifying and reviewing the needs of participants/workers across all phases of recovery and community participation.
🎯

Facilitating supports

Facilitating supports to enhance each participant’s/worker’s capacity to achieve their desired goals, as identified in their My Plan.
🤝

Collaborating with stakeholders

Working collaboratively with internal specialist services, external case managers, health professionals, and other key stakeholders to coordinate effective options for treatment and support.

Experience in service coordination

Proven experience coordinating services for individuals with disabilities.
