You'll be responsible for

Working collaboratively in a multi-disciplinary team to deliver intensive case management and manage education initiatives for at-risk students.

Building effective partnerships with stakeholders, families, and education providers to support student engagement.

Implementing best practices

Facilitating and leading the coordination of individual student plans using a culturally appropriate approach.
