You'll be responsible for

Delivering intensive case management

Working collaboratively in a multi-disciplinary team to deliver intensive case management and provide transition supports for students at risk of disengagement.

Establishing partnerships

Establishing and sustaining effective partnerships with stakeholders, including families and community organizations, to support student engagement.

Implementing best practices

Implementing best practice, culturally appropriate case management for all students to improve learning and engagement outcomes.

Skills you'll need

Intensive case management

Demonstrating the ability to deliver intensive case management for at-risk young people, working directly with students to address barriers to education engagement.

Collaboration

Collaborating effectively with school and regional staff, and building strong, trust-based relationships with young people and their families.
