You'll be responsible for

Supporting individuals

Work in partnership with the person being supported, their family, and informal support networks to attain their hopes and aspirations
🏡

Providing personal care

Provide support to people in their everyday living situations, including personal care as identified, with an emphasis on choice and enhancing quality of life

Developing individual plans

Participate in the development, implementation, and review of individual plans that focus on the life hopes and aspirations of people

Demonstrated experience working effectively in team-oriented environments.
