You'll be responsible for

Supporting individuals

Work in partnership with the person being supported, their family and informal support networks, to attain the hopes and aspirations they have established for their lives
🛁

Providing personal care

Provide support to people in their everyday living situations, including personal care as identified, with an emphasis on choice, enhancing quality of life
📋

Developing individual plans

Participate in the development, implementation and review of individual plans that focus on the life hopes and aspirations of people

Team collaboration experience

Demonstrated experience working effectively in a team-oriented environment.

Proven ability to communicate effectively in spoken and written English.

Experience in disability services

Experience working in the Disability, Aged, and Community sector is ideal.
