You'll be responsible for

🏠

Supporting implementation of the Homelessness Stream Model of Care

Contributing to the framework that guides service delivery and expectations.

Performing in line with Trauma Informed Care principles

Ensuring that care practices are sensitive to the needs of individuals who have experienced trauma.
🤝

Collaborating with Team Leaders and Case Managers

Working together to address client needs in a timely and empathetic manner.

Experience with needs-based assessments

Demonstrated experience conducting assessments for vulnerable populations.
